What Is Complex Care?
Complex care is specialist support for individuals whose medical and personal needs require a level of expertise that goes beyond standard care provision. Unlike domiciliary care, which focuses primarily on daily living tasks, complex care is clinically informed — designed for people living with high-dependency conditions that require trained, consistent and closely supervised support.

Who Is Complex Care For?
Complex care is for adults whose conditions create significant clinical risk and whose care needs cannot safely be met through standard support alone. This may include people who have experienced a life-changing injury or diagnosis, or those whose needs have escalated over time as a result of a progressive condition.
We provide complex care for individuals living with:
- Acquired brain injury, including traumatic brain injury, stroke and hypoxic brain injury
- Spinal cord injuries, including high-level cervical, thoracic and lumbar lesions

What Makes Complex Care Different
The difference between complex care and standard domiciliary care lies in the depth of clinical knowledge, the intensity of support, and the degree of coordination required. Complex care packages are not simply larger versions of a home care visit. They are bespoke clinical services, built from the ground up around an individual’s specific medical profile and risk factors.
Our complex care teams are recruited and trained specifically for each individual package. Before a package begins, staff receive role-specific clinical training — which may include ventilator management, suction, tracheostomy care, enteral feeding, bowel management and advanced pressure care — and must demonstrate competency before they begin supporting the individual unsupported.
Every package is overseen by a named care manager with clinical expertise, and we maintain close working relationships with the NHS clinicians, therapists and case managers involved in each person’s wider care.
Our Approach to Building a Complex Care Package
Every complex care package we deliver is built from scratch around the individual. We do not use templates and we do not apply a one-size-fits-all model. The process of building a package involves the individual, their family, and every relevant professional involved in their care — and it takes as long as it needs to take to get it right.
Assessment and Referral
We begin with a thorough, multi-disciplinary assessment — reviewing all available clinical documentation and meeting with the individual, their family and their existing care team. Our goal at this stage is to understand not just the clinical picture, but the person: their personality, their preferences, their daily routine and what matters most to them in their life.
Team Recruitment and Training
Once the assessment is complete, we recruit and build a dedicated team specifically for the package. Staff are selected for their skills, their reliability and their compatibility with the individual they will be supporting. We then deliver a bespoke training programme, including shadowing, skills practice and formal competency sign-off, before any unsupported care begins.
Care Planning and Risk Management
We develop a detailed, person-centred care plan covering all clinical protocols, risk assessments, emergency procedures and communication responsibilities. This plan is shared with the individual, their family and all relevant professionals — and it is a living document, reviewed and updated as needs or circumstances change.
Ongoing Review and Adaptation
Complex care packages rarely stay static. Needs change, conditions progress, and new challenges arise. We review every package regularly with the individual and their wider clinical team, and we adapt quickly when circumstances require it — scaling support up or down, retraining staff and updating protocols as needed.
Round-the-Clock Support
Many complex care packages require support that extends well beyond daytime hours. We provide 24-hour care, waking nights and sleep-in support, structured around the individual’s daily routine and clinical risk profile. Our on-call management team is available around the clock, ensuring that any concerns or emergencies outside of standard hours are dealt with promptly and professionally.

The majority of complex care packages involve some form of commissioned funding — whether through NHS Continuing Healthcare, a joint NHS and local authority arrangement, or an individual personal health budget. We have extensive experience working directly with commissioners, clinical case managers and Continuing Healthcare teams, and we are accustomed to the documentation, reporting and review requirements that funded packages require.
